Visa Requirements

Note: All documents must be original unless otherwise stated.

1. PHILIPPINE PASSPORT

  • Broken passport is not accepted.
  • At least 6 months valid from departure date and have at least two (2) blank pages.
  • Holder's signature required.

2. VISA APPLICATION FORM

  • Application Form Size: A-4 size (21.0×29.7cm)
  • Application form should be filled out all items correctly. If items are not applicable, please fill in [N/A]. Application form without date of application, signature, and with blank item will not be accepted by the Embassy.
  • Please do not use Friction Erasable Pen in Visa Application Form and in all documents.
  • Do not use liquid paper as means for erasures. Corrections must be crossed out with a double line only.

3. PASSPORT SIZE PICTURE

  • Photo size is 45×35mm (4.5×3.5 cm) with white background. Picture must betaken within 6 months.
  • Please write applicant's name and birthday on back side of the photo.
  • Photo must be pasted on the application form.
  • Please only put glue in each back corners of the picture.

*IF THERE IS/ARE USED JAPANESE VISA ON YOUR PASSPORT,
the following 4 and 5 items are unnecessary.


*PLEASE ATTACH YOUR OLD PASSPORT IF THERE IS/ARE USED JAPAN VISA (LANDING PERMISSION) ON YOUR OLD PASSPORT.

4. BIRTH CERTIFICATE (ORIGINAL)

  • It must be issued within one (1) year from PSA Main Office/Serbilis Outlet Center(Nationwide).
  • If the birth certificate from Philippine Statistics Office (PSA) is unreadable, or has incomplete information, please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.
  • If the birth certification is "Late Registration", please submit in addition, a Baptismal Certificate (Original within 3 months from issuance) and School Record (form 137) from high school or elementary and school yearbook. (Original within 3 months from issuance).
  • If there is no record in PSA, please submit a "Certificate of None-Record" from PSA together with a birth certificate from the Local Civil registrar.

5. ORIGINAL MARRIAGE CERTIFICATE (if the applicant is married)

  • It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
  • In case there is no record in PSA, please submit a "Certificate of None Record" from PSA together with the Marriage certificate from the Local Civil Registrar.

6. DAILY SCHEDULE IN JAPAN (TAIZAI YOTEIHYO) ORIGINAL
*IF APPLICANT WILL SHOULDER PART OR ALL OF HIS/HER TRAVEL EXPENSES.

7. BANK CERTIFICATE

  • Balance within the last six (6) months must be shown.
  • Must be valid three (3) months from the date of issuance.
  • If the Average Daily Balance (ADB) in the last six months is not indicated, bank statement must be submitted to prove transactions within the last six months.

8. INCOME TAX RETURN

  • Latest and clear photocopy of Form 2316.
  • Must have signature of employer and employee.
  • For business owners, proof of actual payment of tax mist be submitted in addition to the BIR Tax Payment Certificate Form (copy acceptable).

9. APPLICANT’S EMPLOYMENT CERTIFICATE

  • Must indicate period of employment, salary, and position.
  • For business owners, Department of Trade, and Industry (DTI) “Certificate of Business Name Registration” and Mayor’s Permit from the City Hall must be submitted.
  • If for any reason other than being a retiree, full-time housewife or unemployed, applicant is unable to submit (8) and (9) above, a letter of explanation about the reason must be submitted.
  • For students, a copy of School ID or Certificate of Enrollment in School must be submitted.

*IF GUARANTOR WHO LIVES IN THE PHILIPPINES WILL SHOULDER PART OR ALL OF THE APPLICANT’S TRAVEL EXPENSES:

10. GUARANTEE LETTER (EMBASSY FORM) (ORIGINAL)

  • If Guarantee Letter is of personal format, it must be addressed to “To:(Ambassador/Consul-General) of Japan”.

11. PROOF OF RELATIONSHIP BETWEEN APPLICANT AND GUARANTOR (BIRTH CERTIFICATES OR PHOTOS, ETC.) (ORIGINAL)

12. BANK CERTIFICATE [see (7) above].

13. PHOTOCOPY OF TAX PAYMENT CERTIFICATE [see (8) above].

14. EMPLOYMENT CERTIFICATE [see (9) above].

15.FOREIGN RESIDENTS with long-term or permanent status in the Philippines applying for a visa must submit, in addition to the above requirements (except birth and marriage certificates), a copy of their Alien Certificate of Registration (ACR) issued by the Philippine government.

*Visa application of foreigners who are on short-term (temporary) visa in the Philippines cannot be accepted. Please apply at the Japanese Embassy / Consulate General with jurisdiction over the area in which the applicant resides over the country of which the applicant’s passport was issued.

Additional Requirements

Notes

  • All documents must be in A4 size only. A document of regal size (or almost same as A4 size) is acceptable.
  • If document is other size, please submit copy that is already adjusted in A4 size from its original and without staples or pasting pages.
  • Applicants are exempted from submitting (4) and (5) indicated above if they can submit old/valid passports with used Japan Visa. Unless required as proof of relationship between applicant and guarantor.
  • It is applicant’s responsibility to ensure that he/she meet the requirements for the grant of a visa. Submission of supporting documents not listed above is encouraged (e.g. applicant’s economic or social ties with the Philippines, urgent reasons for visit: medical certificate, wedding invitation).
  • Passport must have a validity of more than six (6) months from date of departure.
  • The approval of visa application and the granting of multiple entries are subject to the sole discretion of the Embassy.
